Can someone explain this javascript questionnaire framework to me?

I want to create a questionnaire that will give you responses based off the answers you select to a question.

I googled and found this http://ift.tt/1WEAYmr Which sounds exactly like what i want. But when looking through it, I'm not sure how it works.

Where do I actually put the questions and answers? When I run the file called index, I get a page with what looks like missing resources and a link back to the site where he says he implemented it at.
