Google keeps indexing wrong links. HTAccess problem?

The search console (Google webmaster tools) crawl errors section reports new crawl errors each day. Errors that should not be there as the URL's were never created/submitted.

The strange thing is that Google seems to loop through the language pages and add another language layer (/es/es/ and /en/en). Why does this happen? The errors keep piling up. Help!
