Breaking out of being a "good" designer to a "great" designer?

My professor Rayce just gave a fantastic speech about how "good" designers see other people's problems and come up with solutions for them (whether it's in the entertainment industry or elsewhere), and they get the jobs as graphic designers, 3D modelers, people like that. But "great" designers see things that nobody else does. They're the ones who start as 3D modelers/graphic designers and then make their way to the top of the industry, being lead developers, coming up with all sorts of cool things.

He said, "What happens when you go out there looking for a job, and your portfolio looks the same as everybody else's? What happens when you realize that you're stuck in the same place for the rest of your life? How do you break out of that?"

I'm curious what other people have experienced in this area and how you can change from being a "good" designer to a "great" designer...because I'm pretty stressed and scared now.
