How do you guys make sure your time is covered when billing hourly?

We present a proposal (estimate) at the beginning of a project that clients can add to or remove from; once they place their deposit, that is the contract.

We've been running into clients removing sections from the proposal and inevitably asking "so while I've got you here, what do you think about..." (that was removed from the proposal).

If we bill that time, which is usually significant, the client is upset because they removed it officially though we did the work. Often, we're just out the time.

How do you handle hourly clients?
