Looking for file submission software.

Hey all,

I've been looking around for quite some time now, for a product that allows the end user to select a service, lets say Business Cards, it gives them a price, they enter their details and upload their artwork. It could be something similar or more intricate.

If I'm too vague, let me know and I'll try and explain in more depth. Im sure something exists, I'm just not using the right combination of words to find it.
