Client Questionnaires

Hey web designers,

I'm wondering if anyone uses (and would be willing to share) a stock list of questions to get an idea of what a client is looking for with their project. I want to be able to provide a list of questions that will provide me a basic framework for what they're looking for as far as aesthetic design and functionality.
